JPMP Membership Application

The purpose of this form is to submit information regarding yourself and your applicable completed or in-progress Jurassic Park™/Jurassic World™ Vehicle build. All information on this form should be filled out truthfully, information contained herein will not be made public without your prior consent. This information is for record keeping purposes which allow Motor Pool leadership to gauge human resources, vehicle trends, and provide a method of contact for emergencies and event invites.

Birthdate:

You must be at least 18 years old to apply for membership.

Location:

This information is used to display completed build vehicle locations on a world map. You may opt out of showing personal details, such as your name and social media information. Your email address, phone number and birthdate will never be publicly shown. If you choose to have your personal details hidden, only your vehicle type and JP number will be shown, if approved as a completed build.

Social Media:

For all social media accounts, enter your username without the @ symbol (ex. @Delta347 would be entered as "Delta347")

Vehicle Information:

Please tell us about your primary JP Vehicle. You will be able to add more later.

Vehicle Photos:

At least 1 photo is required. Please upload photos that best show your vehicle.
Include multiple angles (hood, side, rear) to show all graphics and details (if in progress).
